SPRING SPORTS PAPERWORK INFORMATION Any athlete trying out for winter sports at North Point must be eligible and have the proper paperwork turned in. Here is an overview of the Eligibility Paperwork Process: ACADEMIC ELIGIBILITY •Minimum 2.0 GPA from 2nd Quarter 2013-14 Academic Year •No more than 1 Class Failure from 2nd Quarter 2013-14 Academic Year •No more than 4.5 absences from 2nd Quarter 2013-14 Academic Year REQUIRED PAPERWORK •1 Pre-Season Athletic Eligibility Clearance Form (paper w/ yellow duplicate attached) •1 Parental Consent Form (4-page packet…3rd page is for Athletic Physical) •2 Emergency Care Cards (only 1 is necessary if the athlete played a Fall sport) These forms should be delivered, IN PERSON, to the Athletic Director at the designated turn-in times (see below). • Activity Bus Permission Form TURN THIS FORM IN TO THE COACH ATHLETIC PHYSICALS AND ImPact TESTS 1. In order to be eligible, an athlete must have a valid ATHLETIC PHYSICAL on file with the Athletic Director. Physicals are valid for 1 calendar year from the date of exam. 2.
